Phimosis is defined as the inability to retract the skin (foreskin or prepuce) covering the head (glans) of the penis.

A short frenulum (also known as frenulum breve) is when the frenulum of the penis is so short that it restricts the movement that the foreskin can make – the frenulum should be long enough to allow the foreskin to retract smoothly, but a short frenulum often makes this impossible. This causes discomfort during sexual intercourse.

The main symptom is difficulty pulling back the foreskin, as well as pain and tension in the penis. This pain increases during sexual intercourse and can tear and cause bleeding because of the concentration of blood in the area.

There are 4 options with regard to treating a tight or torn frenulum:

1. Observation: No specific treatment in mild cases. You can use a lubricant during sexual activity to avoid the problem from happening again
2. Steroids cream: Steroid cream can resolve the problem in the early stages of scarring. This will need to be prescribed by your doctor.
3. Penile Frenuloplasty is simply a foreskin releasing operation. An incision is made transversely on frenulum and re-stitching vertically lengthwise, thus allowing the full release of the frenulum. This will allow you to keep your foreskin, but at the same time to avoid further trauma to the frenulum.
4. Circumcision means that the foreskin is removed especially in the association of the tight foreskin with or without BXO. The benefit of circumcision is that once the foreskin is removed then you will not have any issue related to it. However, the head of the penis is exposed permanently after the circumcision which is an irreversible procedure.
